FAQs – Transition of Cardiology Services
Important Update
As part of our ongoing efforts to enhance services and strengthen relationships with local specialists, Florida Health Care Plans (FHCP) is transitioning from an employed staff model to contracting for cardiology services with a network of community providers. This strategic shift aims to ensure our members continue to have access to high-quality, high-value care.
What Does this mean for FHCP Members?
Effective March 6, 2026, FHCP will no longer provide cardiology services at our care center locations, except for a limited time at one location in Palm Coast, which will continue to offer services until the end of May 2026. Members who had a recent visit with an FHCP cardiologist were first notified by mail on Feb. 6.
Ensuring Continuity of Care
Striving for uninterrupted care is our highest priority and our clinical teams have taken steps to ensure a seamless transition. A list of impacted members has been distributed to our staff PCP offices to review. We have communicated with network community PCPs to notify them of the transition timeline, offering guidance on how to engage members with a network cardiology partner, and providing them with resources if encountering any barriers. We are committed to ensuring a seamless transition and minimal disruption to our members' care.
Support and Resources for Cardiology Patients
Members with questions about appointments or benefits can contact our Member Services Department at (877) 615-4022.
For prescription refills, our pharmacy team is proactively reviewing cardiology medications to identify patients who will need refills within the next 90 days.
Finding a New Provider
Members affected by the cardiology transition have been advised to contact their PCP to discuss ongoing needs for cardiology services. This enables the PCP to review medical history, provide guidance and recommendations, and place a new referral to a network of community cardiologists. In addition, our Member Services team is available to assist any member who needs help navigating this process. - When the six-digit member number is entered into the Member ID Field, the auto search will populate providers from closest to farthest within the member’s designated network by the member’s home zip code.

Accessing Medical Records
Members can request a copy of their medical records by visiting FHCP.com to access the Authorization to Release PHI form in the Document Center. Alternatively, members can email medrecroi@fhcp.com or call (386) 238-3286 or (386) 676-7171.
