FHCP has adopted a population health focus aimed at helping our members get healthy, stay healthy, and manage existing chronic diseases. As part of our Population Health Management (PHM) strategy, we evaluate the needs of our entire membership to connect you with PHM programs and services tailored to your needs. Our PHM programs include:

  • Annual Flu Shots
  • Diabetic Retinal Exams
  • Breast Cancer Screening
  • Virta – Diabetes and Weight Management
  • Emergency Room Utilization
  • Antidepressant Medication Management

Member Education & Resources

FHCP offers members a variety of programs and services focusing on prevention, early detection, and management of diabetes and other conditions at little or no cost. Members are not limited by the number of programs they may participate in and flexible class options are available to accommodate working adults. Classes such as Healthy Heart, Pre-diabetes, Eat Right Move Right, Pregnancy and Nutrition, Kidney Disease and Nutrition, Introduction to Plant Based Eating are some of the classes offered.

The FHCP team is dedicated to listening to and serving our members' needs. Our professional staff help empower individuals with tools, skills, and techniques necessary to meet the challenges of their conditions.

Educational Program Descriptions

  • Acute Low Back and Neck Pain This physical therapy program helps members manage acute or chronic low back or neck pain. For more click here
  • Case Management - Coordination of Care This is a free program offered to members who may benefit from assistance with coordinating their medical, psychosocial, and financial needs. For more click here
  • Diabetes Education Program Recognized by the American Diabetes Association (ADA), and conducted by FHCP registered nurses and registered. For more click here
  • Diabetes Prevention Program This free, two-hour class is designed to help members identify risk factors for developing diabetes.For more click here
  • Healthy Heart Nutrition Program This free class helps members identify risk factors for heart disease and offers tips for improving lifestyle to reduce those risks. For more click here
  • Hypertension (High Blood Pressure) FHCP offers a two-hour hypertension self-management class taught by Registered Dietitians. For more click here
  • Nutrition Game Plan for Diabetes This two-hour session is recommended for members who have completed the IQ-hour Diabetes Education Program. For more click here
  • Osteoporosis This program is for members who are at risk for or have been diagnosed with Osteoporosis or Osteopenia. For more click here
  • Eat Right, Move Right Eat Right, Move Right is a free six-week course that promotes a lifestyle change approach to weight loss for members with a Body Mass Index (BMI) over 27. For more click here
