Sharing Your Health Information

Your health records, more connected care

Starting January 1, 2027, you’ll have more ways to securely share your health information. Give your doctors a clearer picture of your health, bring records from other health plans together, or use a health app to keep your information handy.

These options are available to members with a Medicare Advantage plan or a qualified Health Insurance Marketplace plan.

Bring your health records together

If you’ve had health insurance with another company or have coverage with more than one company now, you can ask Florida Health Care Plans to request your records.

Sharing these records can help your doctors understand your medical history and support your care when you change health plans or doctors.

Your records may include:

  • Medical information from the past five years.

  • Dental and vision records, if those benefits were part of your health plan.

  • Pre-approval information, also called prior authorizations, including approvals, denials, and related records.

Only participating health insurance companies can share records through this service.

How to request your records

  1. Log in to your Florida Health Care Plans member account.

  2. Go to My Account, then Sharing my health data.

  3. Open Data-Sharing Overview and select Health Insurance Company.

  4. Search for your past or other current health insurance company. Have your member ID from that company ready.

  5. Check the consent box to authorize Florida Health Care Plans to request your records.

Repeat these steps for each company you want to request records from. You can return to your account to check your request’s status or request additional records.

Help your doctors see your health history

Sharing your records helps your doctors and care team understand the care you’ve received, even when it came from different doctors or health plans.

Beginning January 1, 2027, sharing with your treating doctors will be turned on automatically. For new members, this setting turns on within one week of the policy’s start date.

The information available may include your Florida Health Care Plans health records, pre-approval history, and records you’ve asked Florida Health Care Plans to collect from other insurers.

You can change your settings

You can turn this sharing off or back on at any time:

  1. Log in to your Florida Health Care Plans member account.

  2. Go to My Account, then Sharing my health data.

  3. Open Data-Sharing Overview.

  4. Select Healthcare Providers to review and change your setting.

Turning off this setting stops sharing through this service. Your doctors may still receive information needed for your care through other methods allowed by law.

Keep your health information handy with an app

You can choose a registered third-party app to view your health information on your phone or another device.

Starting in 2027, the information available through these services will also include pre-approval details. These may show what care was approved or denied, when a decision was made, and related information.

Before connecting an app, review its privacy policy so you understand how it uses and shares your health information.

Questions about sharing your records

When can I request records from another insurer?

Beginning January 1, 2027, you’re eligible from the first day your Florida Health Care Plans coverage starts. You can authorize a request once you’ve set up your member account. You can also call the number on the back of your member ID card for help.

Can I change my sharing choices?

Yes. You choose which participating insurers Florida Health Care Plans can request records from.

For another insurer you currently have coverage with, you can stop or restart ongoing sharing at any time. While sharing is active, Florida Health Care Plans requests updated records about every 90 days.

For a past insurer, a request cannot be canceled after you authorize it. You can choose whether to request additional records.

Can I manage records for my child or another family member?

Parents or guardians can act as personal representatives for their minor children. Call the number on the back of your member ID card to set up permission.

Members age 18 and older must manage their own records through their member account unless someone is authorized to act as their personal representative. This includes spouses and adult dependents on the same policy.

What if my health information is incorrect?

Call the number on the back of your member ID card for help requesting a correction to your records, including information displayed in a connected app.

Does sharing a prior authorization mean my treatment is approved?

Sharing your pre-approval history helps your doctors understand earlier treatment decisions. If you have questions about approval for your current care, call the number on the back of your member ID card.
