Still haven’t received your FHCP Membership Card?

Due to a nationwide paper shortage and a systems issue at our print vendor, it is possible that you may not have received your 2022 membership packet, which includes your FHCP ID card. In an abundance of caution, we have been sending temporary membership cards to those that we think may not have received theirs. If you haven’t already received your official 2022 membership packet, you should have recently received, or will receive soon, a temporary FHCP ID card soon followed by your full packet.

Also, FHCP’s Member Portal is available at https://www.fhcp.com/member-login/ or you can download the myFHCP mobile application to view your plan and benefit information, eligibility history, medications, claims, and authorization requests. You can print or view a temporary ID card, or change your PCP (Primary Care Provider). You can also request a prescription refill at one of our FHCP Pharmacies, view/download/print your benefit plan documents, and securely message FHCP Member Services.
