ACA Rewards Program

With FHCP Rewards, you can earn up to $100 toward your premium when you make healthy choices and participate in exciting wellness programming. Don’t miss out! Get rewarded for your smart healthcare choices. The FHCP Rewards program is available to members 18 and up who have an FHCP Individual ACA Plan.

START EARNING REWARDS

  • Log in to your member portal account. If you do not have a member portal account, you will need to create one.
  • Click the banner on the main member portal page to download the FHCP Rewards App from the App store. You will need a Rewards App Account to receive rewards.
  • Every quarter complete the activities available.

Once you complete the quarterly activities, $25 will automatically be applied to your health plan premium the following quarter. Complete all four quarter programs and earn $100!

HOW TO EARN FHCP REWARDS
  • Schedule an appointment with your primary care provider and begin your journey to better health.
  • Enroll in a free FHCP class to learn about healthy eating, heart health, or speak to an FHCP registered dietitian about proper nutrition and developing healthy eating habits.

*You could be eligible for a medical expense gift card if the earned reward amount is greater than your annual premium. The maximum amount of a medical expense gift card is $100 per contract holder each calendar year. Rewards are only available on individual ACA plans. Anyone 18 years or older covered under the health plan, including dependents, is eligible to participate. The medical expense gift card may only be used at designated locations.
