This directory provides a searchable alphabetically listing of Florida Health Care Plans' providers by provider type. You can choose your Primary Care Physician from any PCP (General Practice, Internal Medicine, Family Medicine, or Pediatrics) that is most convenient for you

Some plan providers may have been added or removed from the list after publication. To get the most up-to-date information about Florida Health Care Plans providers in your area, you can call our Member Services Department at 386-615-4022 or 1-877-615-4022. Hours of operation are 7 days a week, 8 am to 8 pm. TTY users should call TRS Relay 711.
