Florida Health Care Plans Case Management Coordination of Care Program is a free, voluntary program offered to FHCP members who may need assistance coordinating their physical, psychosocial, health related, and/or financial needs.
Our Nurse Care Coordinators are experienced in assessing member needs and have developed a strong network of community resources. Working together with you and your physician, the Nurse Care Coordinator's primary goal is to provide the education and resources needed for you to better understand your health condition(s) and reinforce the plan of care developed by your physicians.
A few helpful services we may identify include:
- Pharmacist Review of Medications for possible cost reduction
- Diabetic Education
- Disease Management Programs
- Support groups
- Home Health and/or hospice care needs
- Community programs for financial assistance with health care needs
