Seeing a Doctor Out-of-Network

Florida Health Care Plan, Inc. (FHCP) has formed a network of doctors, specialists, and hospitals. You can use any doctor who is part of our network. Florida Health Care Plan, Inc. also offers an optional point-of-service (POS) benefit. If you choose this optional POS benefit, you may choose to see any physicians or utilize facilities that are not in FHCP's network of providers without a referral, please keep in mind that when deciding to go to an out-of-network physician that the Optional POS benefit is limited to providers that accept Medicare assignment only. WHAT HAPPENS IF I GO TO A DOCTOR WHO IS NOT IN FHCP's NETWORK?

If you choose the Optional Point-of-Service benefit, you will be responsible for a 20% coinsurance for all out-of-network physician or provider services with the exception of Inpatient Hospital - Acute Care facility or Inpatient Psychiatric Hospital where a $200 copay per day for days 1-10 applies. Please keep in mind that, when deciding to go to an out-of-network physician or facility that the Optional POS benefit is LIMITED TO PROVIDERS THAT ACCEPT MEDICARE ASSIGNMENT ONLY.
