Florida Health Care Plans will cover home infusion therapy if:

• Your prescription drug is on Florida Health Care Plans' formulary or you have an exception;
• Florida Health Care Plans has approved your prescription drug for home infusion therapy;
• Your prescription is written by an authorized prescriber;
