Diabetes Program

Diabetes is a chronic illness that requires continuous medical care and education in order to prevent complications, and to reduce the risk of long-term medical problems. A comprehensive self-management education program is critical in the treatment of this disease. The goal of the FHCP program for diabetes is to provide patients with the tools, skills, and techniques necessary to follow the prescribed treatment of your physician. We want you to follow the treatment regime safely, comfortably, and accurately, ensuring blood sugar control through self-monitoring, nutrition, and exercise habits. Furthermore, the Diabetes Team wants to provide all of our patients with quality diabetes self-management education, encouraging active participation in your own health care. FHCP Nutrition and Nurse Educators (Certified Diabetes Educators, C.D.E.) conduct a 10-hour Education Program approved by the American Diabetes Association. This is a 3 class series offered in Daytona Beach, Port Orange, Palm Coast, and Orange City. Some of the topics included are: Diabetes overview, complications, signs and symptoms of high and low blood sugar, lifestyle modifications, medications, monitoring guidelines (HbgA1C, blood glucose meters, blood pressure, weight), and foot, skin, and dental care.
