School Immunization Requirements

Be sure to call early to schedule your child for his/her immunizations required for entry into school. The school will send your child home if he/she does not have a valid "Blue Card".

Kindergarten Requirements
  • Diphtheria, Tetanus, Pertussis Series
  • Hepatitis A Series*
  • Hepatitis B Series
  • 2 - Measles, Mumps, Rubella (MMR)
  • Polio Series
  • 2 - Varicella (Chickenpox)
7th through 12th Grade
  • Tetanus Diphtheria Booster
  • Meningococcal*
College Freshman
  • Meningococcal Vaccine

*Not a State of Florida School requirement but a Center for Disease (CDC) recommendation
