Plans and Benefits

Florida Health Care Plans provides health care services to members who live or work in our service area of Volusia, Flagler, Seminole, and Brevard Counties. These health care services are provided through the following channels depending on the members ’ needs and eligibility.

Group Health Plans

• Large Employer Groups (over 50 employees)
• Small Employer Groups (2-50 employees)

Medicare

• Medicare eligibles over the age of 65 that are eligible for Part A and enrolled in Part B
• A Medicare eligible that is disabled and eligible for Part A and enrolled in Part B.

For agents/brokers that are appointed to sell FHCP Medvantage products, eligible Medicare beneficiaries may apply for the following FHCP Medvantage plans:

• Medvantage Rx Plus (HMO-POS)
• Medvantage Rx (HMO)
• Medvantage (HMO-POS)
• Premier (HMO)
• Premier Plus (HMO-POS)

Individual Plans

FHCP Individual Health Insurance Plans are available beginning with an effective date of 1/1/2016.
