Florida Health Care Plans provides health care services to members who live or work in our service area of Volusia and Flagler Counties. These health care services are provided through the following channels depending on the members ’ needs and eligibility.
Group Health Plans
- Large Employer Groups (over 50 employees)
- Small Employer Groups (2-50 employees)
- One-Life Groups (Self employed - no other employees)
Medicare
- Medicare Member must apply directly with Florida Health Care. Brokers are not authorized to write FHCP Medicare coverage. However, FHCP offers a Medicare Referral Program and appreciates your business. Please complete a Medicare Referral Compensation Agreement and a Medicare Referral Form if you would like to be a part of FHCP’s Medicare Referral Program.
- Medicare eligibles over the age of 65 that are eligible for Part A and enrolled in Part B
- A Medicare eligible that is disabled and eligible for Part A and enrolled in Part B.
Florida Healthy Kids
Eligible Families must apply directly for coverage. For complete eligibility, benefit, and application information please visit www.healthykids.org.
COVER FLORIDA HEALTH ACCESS PROGRAM
Florida Health Care Plans (FHCP), a state certified, federally qualified, health maintenance organization (HMO) Accredited by URAC, is pleased to offer the Cover Florida plan to residents of Volusia and Flagler Counties. FHCP welcomes you to the Cover Florida program.
THIS IS A LIMITED BENEFIT PROGRAM.
The Cover Florida program is intended to expand the availability of health care options for uninsured residents by developing an affordable health care product that emphasizes coverage for basic and preventive health care services. Many services normally offered under a health plan will not be included in this coverage, or will have coverage limitations. It is important that you read through the entire Cover Florida handbook and refer to the benefit sheets to understand these limitations and restrictions.
