We need to know: How was your experience with your health care provider? Tell us and we'll be better able to help you make the right choices. When you receive your "Welcome to FHCP" member packet, please fill out the survey postcard with postage already paid and return. Your valued feedback will help us manage both the quality and cost of health care.
